The standard rate for therapy is $120 per 50 minute session. To increase accessibility to services, Mosaic Family Services offers an income-based sliding scale for self-pay therapy sessions. For more information, call (904) 527-5202, extension 2.
Payments
Payment with credit card is collected at the end of each telehealth session; in-person clients may also pay with cash or check.

New clients will be able to verify in-network coverage when scheduling their first appointment.
Some clients may also have out-of-network benefits that reimburse payments for mental health services. If so, Mosaic Family Services can provide clients a Superbill (Statement for Reimbursement) to submit directly to the insurance company. Superbills are provided only for services paid for at the standard rate.
Important: in order to submit a Superbill to insurance, a mental health diagnosis is required and will become part of a client’s permanent medical record. If you have any questions or concerns, please contact Mosaic Family Services.
24-Hour Cancellation Policy
It’s never a problem to cancel or reschedule an appointment with at least 24 hours notice. If less than 24 hours notice is given, Mosaic Family Services will charge $75 for the appointment. However, clients with symptoms of illness or other Covid-19 related concerns may request to change sessions from in-person to telehealth.
